Meals on Wheels (Home Delivered Meals)

Home Delivered Meals provides a nutritious meal and personal contact to homebound seniors in the San Joaquin County community. A 5-pack of frozen meals is delivered to your door weekly, along with fresh milk and fruit. Meals are designed to be low fat, low salt, and low sugar. Meals are delivered once a week by 2 p.m. Cost: There is a $2.00 per meal suggested contribution. No one will be denied services for the inability to contribute. Application Process: For more information, call (209) 468-1104 or you can use the Service Referral and Follow-up Summary form (SJ30A) to refer yourself or someone else. Fax the completed Sections 1-4.
