Stockton PACE

Stockton PACE helps seniors unlock their full potential and live at their very bestwith confidence, purpose, and joy. We do it through a model called PACE (Program of All-Inclusive Care for the Elderly), a long-standing program designed to improve care and quality of life for frail seniors. With our support, most participants are able to live safely and more independently in their communities rather than in a nursing home. PACE is a health plan; Most participants have Medi-Cal coverage.
